If you blocked access, click the lock icon (site settings) left of the address bar, allow the camera and try again. It may also fail if a video call app is using the camera, and browsers block cameras on non-https pages.
ASCII art only lines up in a monospace font where every character has the same width, so switch your notes app or chat to a monospace font. Block and dot characters can vary in width between fonts, so Classic or Detailed works best as text.
If your device has more than one camera, a Switch camera button appears below the view. Mirror turns off automatically when you switch to the rear camera.
